Please send electronic copies if possible. If the fees will be more than $[amount], please contact me before doing the work.

Re: Cancellation of purchase made on [date of purchase], [description of product or service], [contract or order number]

I am cancelling this contract. Utah law gives buyers in certain direct sales the right to cancel before midnight of the third business day after the transaction or receipt of the product, whichever is later (Utah Code § 13-11-4).

Please refund all payments to me within 10 business days and tell me how to return any product. Do not charge my card or bank account any further.

Re: Account [number], service contract dated [date]

I am cancelling this service. I did not receive the written notice of the automatic renewal provision that Utah Code § 15-10-201 requires (on the first page of the contract and again 30 to 90 days before the deadline to give notice). Under Utah Code § 15-10-202, the automatic renewal provision is void and the contract renews only month to month.

Please confirm in writing that the contract ends at the end of the current month and that no further charges will be made.
